Consent Form

Womankind Physiotherapy collects information from you for the primary purpose of providing quality health care. We require you to provide us with your personal details and a full medical history so that we may properly assess, diagnose, treat and be proactive in your health care needs. This means we will use the information you provide in the following ways:
1. Administrative purposes in running our health practice.
2. Billing purposes, including compliance with Medicare and Health Insurance Commission requirements.
3. With your written consent (on first page of this document), disclosure to others involved in your health care, including treating doctors and specialists outside this medical practice. This may occur through referral to other doctors, or for medical tests and in the reports or results returned to us following the referrals.
4. Womankind Physiotherapy uses the medical software system ‘Cliniko' for storing patient information and treatment notes. Cliniko is an ultra-secure cloud-based software. Data is encrypted during transfer using a 2048-bit SSL certificate and the database is encrypted at rest using industry standard AES-256 encryption algorithm.

Cancellation Policy
Please let us know if you need to reschedule an appointment at your earliest convenience so we can allow other patients to book in as required. A minimum of 24 hours notice is required for all appointment changes. Cancellations made within 24 hours will incur 100% of the appointment fee. We appreciate your understanding.
